How to Refer

• Kindly obtain your patient’s permission prior to referral.

• Complete the submission form below; Or, fax referral information and records to (833) 764-4009.

  • Consult — Diagnostic clarification and/or treatment recommendations.

  • Evaluation and treat — Ongoing psychiatric care including evaluation, treatment planning, and follow-up visits.

• I typically reach out within 1–2 business days, with most new patients scheduled within 1–2 weeks.
